Position Description:
Full-time, salaried exempt position.
Essential functions include:
- Completing Quarterly Medical History Reviews
- Attending staff and individual support plan meetings
- Attending hospital/rehab discharge meetings
- Providing staff training and education
- Triaging calls outside of normal working hours
- Ensuring the health, safety, and welfare of individuals with a primary diagnosis of MH/IDD
- $1,000.00 SIGN-ON BONUS, payable upon completion of 90 days of employment
Specific Requirements and Preferences:
- Must be licensed as a practical nurse in the State of Pennsylvania
- Minimum of 3 years post-licensure experience required
- Basic computer skills
- Must possess a valid driver’s license and at least 3 years of good driving experience with your license, and must meet our driver eligibility criteria (listed on the application)
- Direct deposit must be established within 90 days of hire date as a condition of continued employment
